Any person whose name has been placed on the inactive list may be restored to active status upon the filing of a written request for reinstatement of license, accompanied by the reinstatement fee as established by the board, proof of satisfaction of continuing medical education requirements established by N.H. Rev. Stat. § 329:16-g, and such other evidence of professional competence as the board may reasonably require.
