The department of health and human services shall record the following information for a “foundling,” which shall mean an infant of unknown parentage: the name given to the child, if available; the date and place of filing; the child’s sex; the approximate birth date of the child; the signature and title of the custodian; the name and address of the person or institution with whom the child has been placed for care by the department of health and human services; other available data regarding the physical condition of the child, as determined by approximation, in accordance with N.H. Rev. Stat. § 5-C:19; the notation “foundling;” the city or town where the child was found, which shall be entered as the place of birth; and parentage data, which shall be shown as “unknown.”
