A. All dental licenses issued by the board shall bear: (1)     a serial number;

(2)     the full name of the licensee; (3)     the date of issue;

(4)     the seal of the board;

(5)     if the license is a specialty license, the specialty to which practice is limited;

(6)     the signatures of a majority of the board members; and

(7)     the attestation of the board president and secretary. B. All dental therapy licenses issued by the board shall bear:

(1)     a serial number;

(2)     the full name of the licensee; (3)     the date of issue;

(4)     the seal of the board;

(5)     the signatures of a majority of the board members; and

(6)     the attestation of the board president and secretary. C. All dental hygienist licenses issued by the board shall bear:

(1)     a serial number;

(2)     the full name of the licensee; (3)     the date of issue;

(4)     the seal of the board;

(5)     the signatures of a majority of the committee members; and

(6)     the attestation of the board president and secretary. D. Certificates issued to dental assistants shall bear:

(1)     a serial number;

(2)     the full name of the assistant; (3)     the date of issue;

(4)     the date of expiration;

(5)     the expanded functions certified to perform; and

(6)     the attestation of the board secretary.

E. All licenses and certificates shall be displayed in a conspicuous place in the office where the holder practices. The license or certificate shall, upon request, be exhibited to any of the members of the board, the committee or its authorized agent.
