(1) ‘Certificate’ means a written statement evidencing the coverage of a person insured under a group insurance policy.

- Person: includes individuals, corporations, associations, firms, partnerships, limited liability companies and joint stock companies. See Oregon Statutes 174.100
(2) ‘Certificate holder’ means an employee or member of a group insured under a group insurance policy. [1981 c.752 § 11]
