The undertaking entered into pursuant to § 40-10-107 may be in substance as follows:

H., charged with a public offense, do each bind ourselves to appear at the next term of the (circuit) court of ______________ County, on the second day of the term, to give evidence against G. H., and, in case of failure, to pay the state of Tennessee two hundred fifty dollars ($250).
