Terms Used In Utah Code 20A-5-404
- Ballot: means the storage medium, including a paper, mechanical, or electronic storage medium, that records an individual voter's vote. See Utah Code 20A-1-102
- Election: means a regular general election, a municipal general election, a statewide special election, a local special election, a regular primary election, a municipal primary election, and a special district election. See Utah Code 20A-1-102
- Election officer: means :(23)(a) the lieutenant governor, for all statewide ballots and elections;(23)(b) the county clerk for:(23)(b)(i) a county ballot and election; and(23)(b)(ii) a ballot and election as a provider election officer as provided in Section
Utah Code 20A-1-102 - Official register: means the official record furnished to election officials by the election officer that contains the information required by Section
20A-5-401 . See Utah Code 20A-1-102- Pollbook: means a record of the names of voters in the order that they appear to cast votes. See Utah Code 20A-1-102
- Polling place: means a building where voting is conducted. See Utah Code 20A-1-102
- State: when applied to the different parts of the United States, includes a state, district, or territory of the United States. See Utah Code 68-3-12.5
- Voting precinct: means the smallest geographical voting unit, established under Chapter 5, Part 3, Duties of the County and Municipal Legislative Bodies. See Utah Code 20A-1-102
(1)(a) For each election, the election officer shall prepare, for each polling place:(1)(a)(i) forms for poll workers to record and verify security seals, ballots cast, and the number of voters who voted; and(1)(a)(ii) an official register or pollbook.(1)(b) For each election, the election officer shall:(1)(b)(i) provide a copy of each form to each of those precincts using paper ballots; and(1)(b)(ii) provide a copy of the ballot disposition form and a pollbook to each of those voting precincts using an automated voting system.(2) The election officer shall ensure that the forms described in Subsection (1)(a)(i) include:(2)(a) a space for the judges to identify:(2)(a)(i) the number of ballots voted;(2)(a)(ii) the number of registered voters listed in the official register or pollbook; and(2)(a)(iii) the total number of voters voting according to the official register or pollbook; and(2)(b) a certification, in substantially the following form:“We, the undersigned, judges of an election held at ______ voting precinct, in _______ County, state of Utah, on __________(month\day\year), having first been sworn according to law, certify that the information in this form is a true statement of the number and names of the individuals voting in the voting precinct at the election, and that the total number of individuals voting at the election was ____.

(3) The election officer shall ensure that the official register or pollbook:(3)(a) identifies the voting precinct number on the face of the official register or pollbook; and(3)(b) contains:(3)(b)(i) a section to record individuals voting on election day; and(3)(b)(ii) a section in which to record voters who are challenged. - Official register: means the official record furnished to election officials by the election officer that contains the information required by Section
