(1) The warden of the state prison, keeper of any jail or correctional institution, and superintendent of the state hospital shall forward to the division:

- Department: means the Department of Public Safety. See Utah Code 53-10-102
- Division: means the Criminal Investigations and Technical Services Division created in Section 53-10-103. See Utah Code 53-10-102
- Month: means a calendar month, unless otherwise expressed. See Utah Code 68-3-12.5
- Person: means :(24)(a) an individual;(24)(b) an association;(24)(c) an institution;(24)(d) a corporation;(24)(e) a company;(24)(f) a trust;(24)(g) a limited liability company;(24)(h) a partnership;(24)(i) a political subdivision;(24)(j) a government office, department, division, bureau, or other body of government; and(24)(k) any other organization or entity. See Utah Code 68-3-12.5
- Probation: A sentencing alternative to imprisonment in which the court releases convicted defendants under supervision as long as certain conditions are observed.
- State: when applied to the different parts of the United States, includes a state, district, or territory of the United States. See Utah Code 68-3-12.5
(1)(a) the fingerprints and recent photographs of all persons confined in each institution under criminal commitment;(1)(b) information relating to the parole, termination or expiration of sentence, or any other release of each person from confinement during the preceding month; and(1)(c) a photograph taken near the time of release.
(2) The adult probation and parole section of the Department of Corrections shall furnish to the division:
(2)(a) information relating to the revocation or termination of probation or parole; and
(2)(b) upon request, the names, fingerprints, photographs, and other data.
(3) The chair of the Board of Pardons and Parole shall provide to the division information regarding the issuance, recall, cancellation, or modification of any warrant issued by members of the Board of Pardons and Parole, under Section 77-27-11 , within one day of issuance.
(4) Information provided to the division under this section shall be on forms designated by the division.
