(a) The Bureau shall prescribe the written and electronic forms for reporting all required information regarding patients with tuberculosis.
Terms Used In West Virginia Code 16-3D-5
- Bureau: means the Bureau for Public Health in the Department of Health and Human Resources. See West Virginia Code 16-3D-2
- State: when applied to a part of the United States and not restricted by the context, includes the District of Columbia and the several territories, and the words "United States" also include the said district and territories. See West Virginia Code 2-2-10
- Tuberculosis: means a communicable disease caused by the bacteria, Mycobacterium tuberculosis, which is demonstrated by clinical, bacteriological, radiographic or epidemiological evidence. See West Virginia Code 16-3D-2
(b) The Bureau shall prescribe the written and electronic forms to be used in committing patients to any state hospital or other health care facility where care and treatment of tuberculosis patients is conducted.
