A. Each health care institution licensed pursuant to this chapter shall adopt a procedure for reviewing reports made in good faith by a health professional concerning an activity, policy or practice that the health professional reasonably believes both:

Terms Used In Arizona Laws 36-450.01

  • Health care institution: means every place, institution, building or agency, whether organized for profit or not, that provides facilities with medical services, nursing services, behavioral health services, health screening services, other health-related services, supervisory care services, personal care services or directed care services and includes home health agencies as defined in section 36-151, outdoor behavioral health care programs and hospice service agencies.   • Professional standards of practice: means practicing within the scope of licensure. See Arizona Laws 36-450

1. Violates professional standards of practice or is against the law.

2. Poses a substantial risk to the health, safety or welfare of a patient.

B. The procedure shall include reasonable measures to maintain the confidentiality of the identity of a health professional providing information to a health care institution pursuant to this section.
