(1) Forms. The forms referred to in these rules are hereby adopted for use. Copies of the rules and the forms may be obtained, free of charge, from the Department by writing the Child Labor Office, at Department of Business and Professional Regulation, Farm and Child Labor Program, Child Labor Compliance, 2601 Blair Stone Road, Tallahassee, FL 32399-2212, (850)487-2536, or for emergency situations after regular business hours (850)566-8541. Copies of any form or document required by the Department, the Child Labor Law or these rules which are transmitted by fax are acceptable only if the hard copy is received by the Department within five (5) working days after the fax copy.     (2) Poster. An employer shall comply with the requirements of Florida Statutes § 450.045(2), by displaying, in a conspicuous place, the Child Labor Law poster which is available, free of charge, from the Child Labor Office at the address provided in Fl. Admin. Code R. 61L-2.012
    (3) Federal Regulations. Employers may obtain copies of the federal regulations regarding prohibited hazardous occupations for minors from the Wage and Hour Division of the United States Department of Labor, or from the Child Labor Office at the address provided in Fl. Admin. Code R. 61L-2.012
