Terms Used In Florida Regulations 64B8-3.002

  • Affidavit: A written statement of facts confirmed by the oath of the party making it, before a notary or officer having authority to administer oaths.
    (1) An application fee in the amount of $300.00 for a person desiring to obtain the following:
    (a) A public health certificate, as provided in Florida Statutes § 458.316;
    (b) A public psychiatry certificate, as provided in Florida Statutes § 458.3165;
    (c) An area of critical need license and a limited license, as provided in Sections 458.315 and 458.317, F.S., However, if the person applying for an area of critical need license or a limited license submits a notarized statement from the employing agency or institution stating that the applicant will not receive monetary compensation for any service involving the practice of medicine, the application fee shall be waived.
    (2) An application fee in the amount of $300.00 for a temporary certificate to practice in an approved cancer center, as provided in Florida Statutes § 458.3135, and a temporary certificate to obtain medical privileges for instructional purposes in conjunction with certain plastic surgery or other medical or surgical training programs and educational symposiums, as provided in Florida Statutes § 458.3137
    (3) An application fee in the amount of $200.00 for a person desiring to obtain registration as a resident physician, assistant resident physician, intern, or fellow, and an application fee in the amount of $300.00 for a house physician, as provided in Florida Statutes § 458.345
    (4) The application fee for a person desiring to be licensed as a physician by endorsement, as provided in Florida Statutes § 458.313, shall be $500.00.
    (5) The application fee for a person desiring to be licensed as a physician by examination, as provided in Sections 458.311, 458.3115 and 458.3124, F.S., shall be $500.00.
    (6) The application fee for a medical faculty certificate, as provided in Florida Statutes § 458.3145, shall be $500.00.
    (7) The initial certification fee for any person who is issued a temporary certificate to practice in areas of critical need, temporary certificate for active duty military and veterans practicing in areas of critical need, public health certificate, public psychiatry certificate, or medical faculty certificate and the initial license fee for a person who is issued a license to practice as a physician as provided in Section 458.311, 458.3115, 458.3124, 458.313 or 458.3151, F.S.; or a limited license as provided in Florida Statutes § 458.317, shall be $424.00 with the following exceptions:
    (a) Any person who is licensed pursuant to Sections 458.3115 and 458.3124, F.S., shall be issued two successive one year restricted licenses.
    (b) A resident physician, assistant resident physician, fellow, or intern in an approved postgraduate training program shall pay an initial activation fee of $200.00 during the first year of the licensure biennium and $100.00 during the second year of the licensure biennium.
    (c) In no event shall an initial license be issued if the initial licensure fee and all required documents are not received within 6 months of the date of certification by the Board. In such a case, the certification expires and the individual affected must reapply and requalify for licensure based on the laws and rules in effect at the time of the new application. However, if the person applying for a limited license as provided in Florida Statutes § 458.317, submits a notarized statement from the employing Agency or institution stating that the applicant will not receive monetary compensation for any service involving the practice of medicine, the initial certification fee shall be waived.
    (d) Any person who is certified pursuant to Florida Statutes § 458.3137, shall pay an initial certification fee of $200.00.
    (e) For any person applying for a temporary certificate to practice in areas of critical need, as provided in Florida Statutes § 458.315, and for any person applying for a temporary certificate for active duty military and veterans practicing in areas of critical need, pursuant to Florida Statutes § 458.3151, who submits an affidavit from the employing agency or institution stating that the applicant will not receive monetary compensation for any service involving the practice of medicine, the initial certification fee shall be waived.
    (8) The initial medical faculty certificate pursuant to Florida Statutes § 458.3145, shall cover a period of 2 years from date of issuance.
    (9) The registration fee of a physician providing medical care and treatment in connection with the education of students, residents, or faculty for not more than 180 consecutive days in a calendar year, shall be $100.00.
    (10) The following fee reductions shall remain in effect until February 1, 2027:
    (a) An application fee in the amount of $350.00 for a person desiring to obtain the following:
    1. A license by examination, as provided in Florida Statutes § 458.311
    2. A license by endorsement, as provided in Florida Statutes § 458.313
    3. A medical faculty certificate, as provided in Florida Statutes § 458.3145
    (b) An initial certification fee in the amount of $350.00 for a person who is issued a temporary certificate to practice in areas of critical need, temporary certificate for active duty military and veterans practicing in areas of critical need, public health certificate, public psychiatry certificate, or medical facility certificate and the initial license fee for a person who is issued a license to practice as a physician as provided in Sections 458.311 and 458.313, F.S., or a limited license as provided in Florida Statutes § 458.317
