Employment certificates shall permit employment during the school vacation or outside of school hours. The employment certificate shall be signed by the Regional or District Superintendent of Schools or their duly authorized agents and shall be in such a form as to show on its face the information and evidence required by Section 11 to be filed before the certificate is issued. An original certificate and 3 copies of the certificate shall be issued and the person issuing it shall:
         (i) mail the original to the minor’s employer,

Terms Used In Illinois Compiled Statutes 820 ILCS 205/10

  • Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
  • Guardian: A person legally empowered and charged with the duty of taking care of and managing the property of another person who because of age, intellect, or health, is incapable of managing his (her) own affairs.
  • State: when applied to different parts of the United States, may be construed to include the District of Columbia and the several territories, and the words "United States" may be construed to include the said district and territories. See Illinois Compiled Statutes 5 ILCS 70/1.14

         (ii) send copies to the State Department of Labor and
    
to the minor’s parent or legal guardian, and
        (iii) retain a copy in his files.
