Terms Used In Indiana Code 27-1-45-7

  • covered individual: means an individual who is entitled to be provided health care services at a cost established according to a network plan. See Indiana Code 27-1-45-1
  • facility: means an institution in which health care services are provided to individuals. See Indiana Code 27-1-45-2
  • practitioner: means the following:

    Indiana Code 27-1-45-5

     (b) A facility or a practitioner is not required to provide the good faith estimate if the health care service to be provided to the covered individual is scheduled to be performed within five (5) business days after the health care service is ordered.
