(1) Each application for a license under this subtitle shall be made in writing, under oath or affirmation, in a form the commissioner prescribes.
(2) Each application shall contain the following information:
(a) In the case of an applicant that is a natural person, the name, electronic mail address, and physical address of the residence and place of business of both the applicant and, if applicable, the managing principal;
(b) In the case of an applicant that is a partnership, limited liability company, or association:
1. Names, electronic mail addresses, and physical addresses of every member and managing principal; and
2. The physical address where the business is to be conducted; (c) In the case of an applicant that is a corporation:
1. The names, electronic mail addresses, and physical addresses of the principal officers, directors, and managing principal; and
2. The physical address of the place where the business is to be conducted;
and
(d) Such additional information as the commissioner prescribes.

(2) Each application shall contain the following information:

- Applicant: means a person filing an application under this subtitle. See Kentucky Statutes 286.4-410
- Commissioner: means the commissioner of the Department of Financial
Institutions. See Kentucky Statutes 286.1-010 - Company: may extend and be applied to any corporation, company, person, partnership, joint stock company, or association. See Kentucky Statutes 446.010
- Directors: when applied to corporations, includes managers or trustees. See Kentucky Statutes 446.010
- Managing principal: means a natural person who meets the requirements of KRS §. See Kentucky Statutes 286.4-410
- Oath: A promise to tell the truth.
- Person: means a natural person, or any type or form of corporation, company, partnership, proprietorship, association, or other legal entity. See Kentucky Statutes 286.1-010
- Statute: A law passed by a legislature.
