(1) Any person who knowingly and willfully violates ORS § 448.415 (2) commits a Class B misdemeanor.

(2) Any person who knowingly makes any false statement, representation, or certification in any application, record, report, plan or other document filed or required to be maintained under ORS § 448.410 to 448.430, or by any rule adopted under ORS § 448.410 to 448.430, commits a Class B misdemeanor.
