The form of subpoena to a witness shall be substantially as follows:

You are hereby required, in the name of the state of Rhode Island, to make your appearance before ………. holden at ………. on the ………. day of ………. to give evidence of what you know relative to an action or plea of ………. then and there are to be heard and tried between ………. plaintiff and ………. defendant.
Hereof fail not, as you will answer your default under the penalty of the law in that behalf made and provided.

- Answer: The formal written statement by a defendant responding to a civil complaint and setting forth the grounds for defense.
- Defendant: In a civil suit, the person complained against; in a criminal case, the person accused of the crime.
- Evidence: Information presented in testimony or in documents that is used to persuade the fact finder (judge or jury) to decide the case for one side or the other.
- Plaintiff: The person who files the complaint in a civil lawsuit.
- Plea: In a criminal case, the defendant's statement pleading "guilty" or "not guilty" in answer to the charges, a declaration made in open court.
- Subpoena: A command to a witness to appear and give testimony.
