(1) Except as provided in Subsection (2), the state board shall annually publish on the state board’s website a report card that includes for each school:

- Parent: means a parent or legal guardian. See Utah Code 53E-1-102
- Process: means a writ or summons issued in the course of a judicial proceeding. See Utah Code 68-3-12.5
- State: when applied to the different parts of the United States, includes a state, district, or territory of the United States. See Utah Code 68-3-12.5
- State board: means the State Board of Education. See Utah Code 53E-1-102
- Statewide assessment: means one or more of the following, as applicable:(2)(a) a standards assessment described in Section 53E-4-303;(2)(b) a high school assessment described in Section 53E-4-304;(2)(c) a college readiness assessment described in Section 53E-4-305; or(2)(d) an alternate assessment administered to a student with a disability. See Utah Code 53E-5-201(1)(a) the school’s overall rating described in Subsection 53E-5-204(1);(1)(b) the school’s performance on each indicator described in:(1)(b)(i) Section 53E-5-205, for an elementary school or a middle school; or(1)(b)(ii) Section 53E-5-206, for a high school;(1)(c) information comparing the school’s performance on each indicator described in Subsection (1)(b) with:(1)(c)(i) the average school performance; and(1)(c)(ii) the school’s performance in all previous years for which data is available;(1)(d) the percentage of students who participated in statewide assessments;(1)(e) for an elementary school, the percentage of students who read on grade level in grades 1 through 3; and(1)(f) for a high school, performance on Advanced Placement exams.
(2) The state board shall collect, but is not required to publish the information described in Subsection (1) related to the 2020-2021 school year.
(3) A school may include in the school’s report card described in Subsection (1) up to two self-reported school quality indicators that:
(3)(a) are approved by the state board for inclusion; and
(3)(b) may include process or input indicators.

(4)(a) The state board shall develop an individualized student achievement report that includes:
(4)(a)(i) information on the student’s level of proficiency as measured by a statewide assessment; and
(4)(a)(ii) a comparison of the student’s academic growth target and actual academic growth as measured by a statewide assessment.
(4)(b) The state board shall, subject to the Family Educational Rights and Privacy Act, 20 U.S.C. § 1232g, make the individualized student achievement report described in Subsection (4)(a) available for a school district or charter school to access electronically.
(4)(c) A school district or charter school shall distribute an individualized student achievement report to the parent of the student to whom the report applies.
